Those with Medical Assistance or MinnesotaCare can now look up online the month of when your health insurance renewal forms will arrive in the mail on the Minnesota Department of Health website. To look up your information, you must enter your case number and member number. You can find your case and member number on either your notices or premium bills. If you still can’t find that information you should call your county or Tribal worker.
Once those renewal forms arrive in the mail, you’re urged to complete and return those documents as soon as possible. Not returning those renewal forms may cause those who are eligible to lose health insurance as keeping your health insurance makes it easier to retain health care.
Your renewal health insurance month is based on when you first got insurance and those who have MinnesotaCare can expect your renewal forms in the mail by November.
